are confidential and will not be released without your
permission.

information regarding visits and treatment at the Dove
Health Alliance/Dove Counseling is completely confidential except as
indicated by state law. Patients must sign a release of
information form if they desire the Dove Health
Alliance/Dove Counseling
staff to consult with anyone, including family members.
